OFFICE HOURS:
Koch Eye Associates is open for scheduled patient visits from 7:30am to 7:00pm, Monday through Friday (hours vary depending upon location, please call to verify); 8:00am to 12:00pm, Saturday (September through May).
The Providence Place Mall Office is open during regular Mall hours.
SCHEDULING AN APPOINTMENT:
If you wish to schedule a patient appointment or consultation, please telephone the office where you wish to be seen, or call our scheduling office at 401-738-4800 or 1-800-KOCHEYE (in Rhode Island, Massachusetts and Connecticut). The scheduling department is open for calls from 8:00am to 5:00pm, Monday through Friday.
CANCELLATION NOTICE:
We require a 24 hour notice for all cancellations unless, of course, an emergency should arise. If appropriate notice of cancellation is not received, you will be billed $25.00 for the time allotted to you.
TRANSPORTATION SERVICES:
Koch Eye Associates will provide transportation in our specially equipped courtesy van to patients who have no other means of transportation.
EMERGENCY CALLS:
We are staffed to receive emergency referrals at any time. Our 24 hour hotline is 401-738-4800 or 1-800-KOCH EYE (in Rhode Island). Calls to either of these numbers will be returned in a timely manner by a staff ophthalmologist or optometrist.
INSURANCE:
Koch Eye Associates accepts Medicare and most major insurance programs. In addition, we are affiliated with many managed health care plans.

KOCH CONTACT LENS, located at 566 Tollgate Road in Warwick, offers patients special care with all their contact lens needs.
KOCH EYE ASSOCIATES, in Providence and Johnston, offer patients an extensive selection of eye wear.
APPLE EYEWEAR, located at 566 Tollgate Road in Warwick, offers an extensive selection of eye wear.
HERITAGE OPTICAL, located on the premises of Koch Eye Associates in Woonsocket, offers patients an extensive selection of eye wear.
WICKFORD OPTICAL, located on the premises of Koch Eye Associates in N. Kingstown, offers patients an extensive selection of eye wear.
