Koch Eye Associates - 566 Tollgate Road, Warwick, RI
From points North, take I-95 South to Exit 10B (117 West). Follow exit ramp onto Route 117 West. At the first light take a right onto Commonwealth Avenue. At the next traffic light turn right onto Tollgate Road. 566 Tollgate Road is the 4th building on the right.
From points South, take I-95 North to Exit 10 (117 East). Follow exit ramp onto Route 117 East. At the second traffic light turn left onto Tollgate Road (Rte. 115). 566 Tollgate Road is approximately a mile up on the left.

Koch Eye Associates - 255 Cass Avenue, Woonsocket, RI
From Diamond Hill Road (Route 114), Travel west until you see Walnut Hill Plaza. Take a left at the light onto Mendon Road. At your second light, turn right onto Cass Avenue. Koch Eye Associates is at 255 Cass Avenue, on the left.
From points South, take I-295 North to Route 146 North, to Route 99. Follow Route 99 to the end and take a left onto Route 122. Follow Route 122 to Cass Avenue. Turn right onto Cass Avenue and Koch Eye Associates is on the right, just past Landmark Medical Ceneter.
From Providence, see above directions except omit the reference to I-295. Take Route 146 North directly from Providence.

Koch Eye Associates - 55 Dorrance Street, Providence, RI
From points North, take I-95 South to Exit 22A (Downtown). At the end of the exit you will come to a traffic light. The Westin Hotel will be on your right and Capital Grille will be in front of you. Take a right at the light. Go straight through the next traffic light (intersection of Exchange Terrace) passing the Biltmore Hotel on your right. This is Dorrance Street. Go through the next 2 traffic lights (city hall on your right). Koch Eye Associates is on the right at 55 Dorrance Street, on the corner of Fulton and Dorrance Streets.

From points South, see above directions except omit the reference to I-95 South. Take I-95 North to Exit 22A.

Providence Place Mall, Level 1 Cityside, Providence, RI
From points South, follow I-95 North to Exit 22C. Follow exit ramp to end and Mall entrance is directly in front of you. Go to Level 1 Cityside and look for the Koch Eye sign.

From points North, take 95S to Exit 22C. Follow directions above.

Koch Eye Associates - 7805 Post Road, North Kingstown, RI
From points North, take I-95 South to Exit 9 (Route 4 South). Take Exit 6 from Route 4. Follow exit ramp onto Route 2 South and stay on Route 2 until you come to a traffic light. At the traffic light, turn left onto Stony Lane. Follow Stony Lane to the 2nd Stop Sign. Turn right and Koch Eye Associates will be on your left, just before Route1.

From points South, follow Route 1 North into North Kingstown. Just past the State Police Barracks (on your left), is Koch Eye Associates at 7805 Post Road, also on the left.

From points South, take 295 North to US-6 exit (Exit 5-6) toward Hartford/Providence.

Keep right at fork in the ramp. Merge onto US-6E. Take RI-5 exit toward Johnston and keep left at the fork in the ramp. Turn left onto Atwood Avenue/RI-5. Our office is on the left at 1404 Atwood Avenue.

From points North, take 295 South to US-6 exit (Exit 6-5) toward Hartford/Providence.

Keep left at the fork in the ramp for approximately .6 miles, and then keep right at the fork in the ramp for approximately .5 miles. Merge onto US-6E. Take RI-5 exit toward Johnston, keeping left at fork in the ramp. Turn left onto Atwood Avenue/RI-5. Our office is on the left at 1404 Atwood Avenue.

From points South, From 95 Take Exit 75 (Flanders). Go straight through the light (Flanders four corners). We are on the left, about 1/8 mile.

From points South, Follow 395 until it turns into 95 South. Take Exit 75 (Flanders). Go straight through the light (Flanders four corners). We are on the left about 1/8 mile.

From points North, From 95 Take Exit 74. Turn left off the exit. Turn left onto Boston Post Road. We are on the left, about 1/8 mile.
